Participant Waiver, Release of Liability & Media/Image Consent
This Waiver, Release of Liability, and Media/Image Consent Agreement (“Agreement”) is entered into by the undersigned participant or, if the participant is a minor, by the participant’s parent or legal guardian, in connection with participation in California Voices Rock, LLC, a community choir organization (“Voices Rock”). By submitting this registration, you acknowledge that you have read, understood, and agree to all terms of this Agreement.
1. Assumption of Risk & Waiver of Liability
Participation in choir activities, including but not limited to rehearsals, performances, concerts, tours, community events, and travel, involves inherent physical risks. These may include, without limitation:
- Physical injury from standing, walking, or movement during rehearsals or performances
- Slips, trips, or falls at rehearsal venues, concert halls, or other facilities
- Vocal strain or hearing-related issues from extended singing or amplified sound
- Injury during travel to or from choir-related events
- Exposure to communicable illness in group settings
- Any other unforeseen accident or injury
I, the undersigned, voluntarily assume all such risks. I acknowledge that Voices Rock, its officers, directors, volunteers, employees, agents, affiliated organizations, and venue owners collectively, “Released Parties”, are not responsible for any injury, illness, loss, damage, or death arising from my participation, whether caused by negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
In consideration for being permitted to participate, I hereby release, waive, and discharge the Released Parties from any and all claims, demands, actions, or causes of action arising out of or relating to my participation in choir activities.
2. Media, Image & Performance Release
I hereby grant to Voices Rock and its authorized representatives a perpetual, irrevocable, royalty-free, worldwide license to photograph, record audio and/or video, film, stream, and otherwise capture my image, likeness, voice, and performance in connection with choir activities.
I authorize Voices Rock to use such materials for any lawful purpose, including but not limited to:
- Promotional materials, including print, digital, and broadcast advertising
- Social media posts, websites, blogs, and online platforms
- Archival, documentary, educational, or historical purposes
- Fundraising campaigns and donor communications
- Press releases, newsletters, and publications
- Public or commercial performance recordings and broadcasts
I waive any right to inspect or approve the finished product or the use to which it may be applied. I understand that I will receive no compensation for the use of my image, likeness, voice, or performance.
I release the Released Parties from any claims arising out of the use of such materials, including but not limited to claims for invasion of privacy, defamation, or right of publicity.
3. Conduct & Participation
I agree to abide by all rules, codes of conduct, and directives established by Voices Rock. I understand that Voices Rock reserves the right to remove any participant from activities for conduct deemed inappropriate or disruptive, without refund of any fees paid.
4. Severability & Governing Law
If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by the laws of the State of California.
5. Acknowledgment
I have read this Agreement carefully. I understand it is a release of legal rights. I agree voluntarily and of my own free will. If I am agreeing on behalf of a minor, I represent that I am the parent or legal guardian of the participant named in this registration and have authority to bind that minor to these terms.
